Please join us for the 6th Annual LCPL Jonathan W. Collins Memorial Fund Golf Outing

to benefit the LCPL Jonathan W. Collins Memorial Fund and "A Message From the Heart^SM".

We will have a great day of golf at the beautiful Gleneagles Country Club in Lemont, IL . The event will include a 4-person best ball, continental breakfast, lunch, dinner and awards reception following the golf.

Registration:	9:00 am	Golf foursome:	$600 ~ $540 if registered before April 15th.
Continental Breakfast:	9:00 am	Individual Golfer:	$150 ~ $135 if registered before April 15th.
